Position: Outreach Worker

Summary of Position:

  • Under the general and direct supervision of the Director of Social Work.
  • Assist in providing ongoing personal and community support to existing members.
  • Serve as liaison between members and available social services, as directed by the Social Worker.
  • Maintain existing client documentation, as well as initiate membership packets for new Center participants.

Responsibilities:

  • Obtain daily absentee member reports and contact each to determine the reason for absence (Note: a visit to the home is required for those members not reached via phone after 2 successive absences, specifically for those members with no phone).
  • Assist members with monthly billing problems associated with utilities (i.e. Gas/electric, Cable).
  • Assist members with all financial sources including, but not limited to, Medicaid, Medicare, SSI and CCPED; food sources including, but not limited to, attainment of food stamps and food vouchers, shelters, food/soup kitchens, food donations; and housing sources, including, but not limited to, low-income housing, shelters, and the Housing Authority.
  • Inform Administration of all hospitalizations of existing members. Coordinate member hospital visits and arrange floral deliveries with Administrator.
  • Complete Home Assessments, as directed, by the Social Worker.
  • Assist in the QA/QC program of member charts (Four charts per day, two members per shift).
  • Provide Center tours for visitors and guests. Initiate the membership packet for incoming members, including completion of the face sheet, arrangement for the physical, attainment of signatures, and procurement of a copy of a valid Medicaid card.
  • Distribute flyers, pamphlets, business cards, etc, promoting 2nd Home while conducting business in the community. (i.e. Home or hospital visits). Assist Director of Marketing, if necessary, with lunches and meetings in the community.
  • Attend staff meetings as scheduled.
  • Make a commitment to cooperative performance to attain care, organization and programmatic objectives which may entail performing duties not included in this job description.

Qualifications:

  • A high school diploma and two years of experience in customer service, community relations, or outreach work.
  • The ability to communicate and write proficiently.
