| Position: Pre-Admissions
Coordinator Summary of Position:
- Under the general and direct supervision of the
Administrator and the Vice President of Program Development.
- Develops and implements plans to increase community
based referral sources.
Responsibilities:
- Coordinate efforts between the center, physicians, and
other referral sources to efficiently evaluate and admit
patients.
- Coordinate marketing and educational efforts with
referral sources.
- Screen referrals for admission into the program.
- Develop a system for coordinating and monitoring
referrals.
- Understand the needs and interests of the elderly in the
community.
- Have a knowledge of the available program slots and days
of availability for additional patients in the center.
- Have a clear understanding of the program and the
medical population served.
- Upon determination of patient eligibility, obtain a
complete physical, conduct the home assessment and complete
the patient referral form.
- Submit all patient documentation to the Administrator to
determine program start date.
- Have a working knowledge of the NJ Department of Health
& Senior Services Standards for Licensure.
- Make a commitment to cooperative performance to attain
care, organization, and programmatic objectives which may
entail performing duties not included in this job
description.
Qualifications:
- Be a New Jersey Licensed Practical Nurse or be a High
school graduate or equivalent, and have prior Case
Management, Discharge Planning or Admission experience.
- Must have a superficial knowledge of the reasons for
client care as provided.
- Must possess a basic proficiency in English, spelling
and math.
- Must be able to pay attention to details.
- Must have reliable transportation.
