| Position: Social Worker Summary of Position:
- Under the general and direct supervision of the Director
of Social Services and the Administrator.
- Implement the social services component of the Day Care
Program.
Responsibilities:
- Interview the clients and caregivers to obtain a
clinical and social assessment and evaluation of needs and
problems and set forth a care plan.
- Provide individual, family and group counseling in
reference to psychological, social, financial, legal,
vocational and educational needs of clients.
- Assist with obtaining personal services such as housing,
shopping, clothing.
- Develop support groups and educational programs for
caregivers and clients.
- Provide crisis intervention.
- Provide family outreach.
- Coordinate recipient’s treatment plans with other
community resources
- Provide in-service training to staff on client/caregiver
psycho-social needs.
- Participate in professional organizations and seminars.
- Participating in all client case conferences.
- Document assessments, treatment plans, evaluations and
clinical notes.
- Attend staff meetings as scheduled.
- Attend Inter Disciplinary Team meetings as scheduled.
- Complete initial home visit or oversee it’s delegation
in processing applicants into the program.
- Make a commitment to cooperative performance to attain
care, organization and programmatic objectives which may
entail performing duties not included in this job
description.
Qualifications:
- Must be certified or licensed by the New Jersey State
Board of Social Work Examiners.
- Must have verification card of current instruction in
CPR.
- Must possess a basic proficiency in English, spelling
and math.
- It is preferable that the individual have experience
working with the elderly with chronic impairments.
